Pediatric Transitional Care Physician (Finance)

Nemours Children's Health is seeking Pediatric Transitional Care Physicians to join our team in Jacksonville, Florida.

The selected candidate will provide comprehensive, developmentally appropriate care for adolescents and young adults with chronic, complex or special healthcare needs. The role focuses on guiding patients and families through the transition from pediatric to adult healthcare, promoting independence, self-management and continuity of care across health systems.

Key responsibilities:

• Provide direct medical care to youth and young adults with chronic or complex medical conditions
• Conduct transition readiness assessments and develop individualized transition care plans in collaboration with patients, families, and providers
• Support coordination of care across pediatric and adult specialties, primary care, behavioral health, and community resources
• Address medical, developmental, psychosocial, and vocational needs during the transition process

Qualifications:

• MD or DO
• Board eligible/certified in specialty
• Eligible for unrestricted Florida medical license and DEA
• Excellent communication and interpersonal skills, with the ability to work effectively as part of a multidisciplinary team
