Job Summary:
Manage medication-related phone calls and emails, coordinating communication between physicians, patients, and pharmacies.Support physician teams by handling medication refill requests and communicating directly with patients regarding their medication needs.

Job Duties / Responsibilities:
Receive and manage refill requests from multiple sources including electronic prescriptions (e-Rx), fax, instant message, phone calls, or walk-ins.Verify patient eligibility for refills by reviewing the last office visit and upcoming appointments.Communication and coordination with Triage nurse or Physician nurse regarding issues or concerns patients might be experiencingCoordinate communication between providers, patients, and pharmacies to ensure timely and accurate medication delivery, maintaining relationships with physicians, nursing staff, and other healthcare providersReview and process medication refill requests in accordance with provider orders and clinical guidelines.Document all communications and actions taken in the electronic medical record (EMR).Monitor for medication interactions, dosing concerns, or refill limitations and escalate issues to the appropriate provider.Provide patient education on medication usage, side effects, and refill procedures.Ensure compliance with HIPAA, infection control, and clinical safety standards.Perform other duties as assigned to support the clinical team and patient care

You'll also handle medication-related phone calls and emails, helping coordinate communication between physicians, patients, and pharmacies.
