What You'll Do:

• Perform wound care and skin assessments per physician orders and facility protocols
• Document treatments and monitor healing progress
• Collaborate with interdisciplinary team to support optimal resident outcomes
• Educate staff on wound prevention and skin integrity best practices
• Participate in care planning and resident care conferences

What We're Looking For:

• Active California LVN or RN license
• Prior wound care or treatment nurse experience in a SNF preferred
• Excellent assessment, documentation, and communication skills
• Compassionate, detail-oriented, and a strong team player
