EIS Case Manager (Education)



Base Salary: $21.50-$32.25 / hour
JOB SUMMARY
The Early Intervention Services Case Manager will assist clients with assessment of need and enrollment and distribution of support services such as delivered meals, transportation services, emergency housing assistance, nutritional supplements and all other services as needed. The coordinator will assist RN Case Manager with referrals for health care and support services needed for core medical and non-medical support services. Will perform the following: screening and enrollment of participants, outreach, assessment, connection to community resources, education, identification of patient centered goals.
JOB DUTIES AND RESPONSIBILITIES
· meet with clients for enrollment in support services, administer required assessments, and review client's care plan, medical follow-up and psychosocial needs. Work collaboratively with RN Case Manager and members of community partner agencies.
Initial assessment of the client's service needs within 30 days of the first visit.
Development of a comprehensive, individualized care plan during the initial assessment visit including client-centered goals and milestones.
Ongoing client monitoring to determine the efficacy of the care plan.
Re-evaluation of the care plan with the client at least every 6 months with revisions and adjustments as necessary.
Ongoing assessment of the client's and other key family members' needs and personal support systems.

· Assist client and RN Case Manager in identifying potential problems/barriers; provide health education resources. Provide advocacy for client and monitor client's progress. Coordinate logistics for health goal adherence
· Assist in timely scheduling of primary care and other medical appointments to support client health. May attend appointments with clients
· Link individuals to the application process for various community assistance programs
· Maintain good working relationship with community health partners by working as a liaison with other agencies and the community
· Document all clinical support tasks to meet data collection requirements
· Participate in quality assurance activities to monitor and improve client service quality
· Assist in locating clients in support of outreach function
· Field work as needed in client's home or during homeless outreach
· Participate in on-going training and reviews
· Must be able to prioritize multiple projects
· Other duties as assigned
KNOWLEDGE, SKILLS, AND ABILITIES
· Proficiency with Microsoft Office applications including Outlook, Word, Excel and PowerPoint
· Ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines
· Strong organizational skills
· Strong written and verbal communication skills
· Ability to effectively present information, respond to questions, and generally communicate clearly and respectfully with participants, coworkers, other agencies and companies
· Comfortable discussing sensitive health issues with clients
· Ability to learn basic health care information and present that to clients
· Strong interpersonal skills. Ability to work with people with a variety of backgrounds and educational levels both in person and on the phone
· Ability to work independently and as part of a team
· Ability to work flexible hours as needed
· Good judgment, problem solving and decision-making skills
· Ability to uphold client confidentiality with the utmost professionalism
· Ability to travel to other offices and community locations
· Support patients in a non-judgmental way.
EDUCATION & EXPERIENCE
· Bachelor's degree from an accredited college or university; may substitute two years of supervised related direct service experience in health or human services.
· Will complete HIV Case Management Training within 60 days of hire and complete annual HIV training as assigned
· Current CA Driver's License
PREFERRED QUALIFICATIONS
· Experience working in a healthcare setting
· Prior case management experience
· Community Health Worker Certificate (CHW), Medical Assistant or Certified Nursing Assistant Certificate (CNA) with one year of community health experience, or a minimum of 5 years of community health/outreach or other relevant experience
