We are currently seeking a skilled Occupational Therapist (OT) to join our team. If you're passionate about patient care and want to make a meaningful impact, we'd love to hear from you!

Position Overview:
As an Occupational Therapist at Santa Rosa Post Acute, you will be responsible for providing individualized therapy services to patients in our skilled nursing facility. You will work closely with patients and interdisciplinary team members to develop and implement therapy plans aimed at improving functional independence and quality of life.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' physical, mental, and emotional needs.
  • Develop personalized treatment plans to improve functional independence in activities of daily living (ADLs).
  • Provide direct therapy interventions, including therapeutic exercises, adaptive equipment training, and cognitive training.
  • Collaborate with the healthcare team (nurses, physicians, speech therapists, etc.) to ensure optimal patient care.
  • Educate patients and families on home safety, adaptive techniques, and equipment.
  • Document patient progress and treatment plans in compliance with facility and regulatory standards.
  • Stay current on best practices and new treatment methodologies in occupational therapy.

Qualifications:
  • Degree in Occupational Therapy from an accredited program.
  • Valid Occupational Therapy license in CA.
  • Experience in a skilled nursing facility or geriatric care setting is preferred.
  • Strong knowledge of rehabilitation techniques, equipment, and therapeutic modalities.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work effectively within a team and independently.
  • Compassionate and patient-centered approach to care.
