Clinic RN (SBHC) (Finance)



Location: Schertz Elementary (757 Curtiss, Schertz, TX 78154)

Essential Duties:
•Supports and practices the mission and philosophy of Methodist Healthcare Ministries
•Utilizing a holistic approach and evidence-based nursing practices, incorporates the nursing process into the goal directed plan of care for the pediatric, adolescent patient.
•Plan of care should include planning and
scheduling of intervention/care and collaboration with other
services/departments. (Patient Care, Nursing, Plan of Care)
a) Prepares and administers various medications and injections;
collects and analyzes specimens as directed.
b) Provides and/or assists with patient care, minor procedures and
treatments within the scope of practice. (Patient Care, Scope of

c) Communicates appointments, specialty procedures, and
instructions by phone, in writing, and in person; coordinates patient
information with other facilities, departments, and staff as
appropriate. (Patient Care, Nursing, Scheduling)
d) Provides education, instructions, and advice to patients as
directed by physician with consideration to age-appropriate care.

e) Responsible for documentation within clinical records (both
electronic or written) of all patient care or treatments provided
(HIM, Patient Chart, Documents in Patient Record)
•Promote and reinforce patient centered medical home concepts with patients and staff
•Works to create, revise and coordinate implementation of the plan of care through collaboration with the multidisciplinary team
•Work with primary care providers in medical services and informatics team to develop a panel of patients and risk stratification via a thorough intake assessment that would benefit Methodist Healthcare Ministries and providers in order to formalize and affirm partnerships and set the stage for all components of The Patient Centered Medical Home (PCMH)
•Develops short term and long-term strategies in improvement of patient outcomes and work with the informatics team to track patient outcomes
•Coordinate and manage the care of these designated patients within the clinic and other primary care departments
•Consult regularly with the behavioral health, dental and medical providers of the identified patients
•Work with providers, staff from other departments, specialists, Care Coordinator, Wesley Nurses, patients and families to facilitate patient through the care system
•Work with outside agencies for welfare checks
•Monitor patients for changes in clinical symptoms both in person and by phone
•Regularly using the nursing process to assess, plan, implement and evaluate the patient's status, appropriate goals, care provided, care interventions, and patient outcomes.
•Administrative work to include membership of the Safety Committee, meeting criteria for Texas Vaccines for Children (TVFC), quality control checks for all equipment used in medical, calling and scheduling with equipment vendor as needed, keeping up with all compliance logs and Methodist Healthcare Ministries systems.

Education and/or Experience:
Must be a graduate of an accredited RN program, with a minimum requirement of an Associate Degree in Nursing (ADN) required. A Bachelor of Science in Nursing (BSN) in health education or case management is preferred. One year of nursing experience in a hospital or clinic setting preferred. Experience in pediatrics or school nursing is preferred.

Certificates, Licenses, Registrations:
Licensed as a Registered Nurse by the State of Texas required. BLS required.

Language Skills:
Must possess strong interpersonal skills, demonstrated in both written and verbal communication. Must be able to think critically, using nursing assessment skills to determine when potential patient safety risks should be reported promptly. Must be able to read, analyze, and interpret medical records, professional journals, technical procedures or government regulations. Must be able to effectively present information and respond to questions from individuals and groups in a culturally and linguistically appropriate manner. English and Spanish required.

Reasoning Ability:
Must be able to analyze data, demonstrated an aptitude for statistics, and efficiently and effectively manage multiple tasks.

Computer Skills:
Must be able to demonstrate working knowledge of the Internet, Outlook, PowerPoint, Word, Excel, and electronic health record applications.
Other:
Must be able to drive and have access to a vehicle; maintain valid driver's license and auto liability insurance. 50% travel as needed.

While performing the duties of this job, the team member is exposed to a medical clinic environment having direct contact with patients.
