The Clinical Coordinator is responsible for assisting with the coordination of patient wound, fistula and ostomy care while providing support and supervision to wound team personnel in the performance of their duties. The Clinical Coordinator is a clinical resource who ensures patients receive the highest quality care by serving as a professional role model, supervising the wound care team's daily operations assuring regulatory compliance and acting as a liaison between management and staff. The CC will provide direct patient care and assume on call responsibilities as determined by hospital needs. The Clinical Coordinator is instrumental in creating a positive public image and working environment. Demonstrates Duties and responsibilities of a Wound care Registered Nurse.Education: ASN required, BSN preferred. Nursing license required.
Experience: Clinical proficiency relevant to specific clinical area. Care Coordination and ambulatory experience required.
Preferred:
Experience in cognitive health or neurology
Experience with ambulatory Epic system strongly preferred
Experience with case management, care navigation preferredJob Summary: The Nurse Navigator will play a crucial role in supporting patients with early signs of cognitive decline. This position involves conducting phone screenings, scheduling necessary tests and MRIs, and serving as an extension of the Primary Care Physician for patients throughout their health journey.
