Occupational Therapist (Healthcare)

Concord Post Acute is seeking a skilled Occupational Therapist to join our team. We have multiple openings available for full-time, part-time, and per diem schedules.

Key Responsibilities

• Develop and execute personalized treatment plans based on physician orders.
• Provide direct patient care to improve daily living skills and independence.
• Document patient progress, evaluations, and certifications in accordance with all regulatory requirements.
• Collaborate with the interdisciplinary team on patient care, progress, and discharge planning.
• Supervise Occupational Therapy Assistants and train Restorative Aides.
• Assess and facilitate ordering of necessary durable medical equipment.
